I remembered coming across how to select a chiropractor for the management of athletic conditions a few years ago but never thought much of it. While preparing the workshop slides for the athletes at Zen Freediving, I started looking into sports chiropractic literature and decided the paper is worth revisiting.

Unfortunately, the paper was published in 2009. By most standards, it is considered pretty outdated. Unfortunately, again, I don’t think there is an updated, newer paper for us to refer to. This is the criteria published in the study and it is loosely based on resources from the Australasian College of Sports Physician Registrars.

  1. Minimum treatment time 15–20 minutes
  2. Treatment is multimodal in nature
  3. Treatment should contain active (exercises) and passive components
  4. No mandatory x-rays required for treatment
  5. No predetermined treatment schedules or prepaid contracts of care
  6. Use of medical terminology and diagnosis

Back in the day, I thought this was pretty good but now I have reservations towards points 1 and 3. I am not sure if 15 minutes – really – is enough time for treatment to include both active and passive components (as per in point 3). Furthermore, I am not sure if this is based on evidence-based recommendations or if this is based on the status quo where most chiropractic consultations are fairly short (15-20 minutes).

As far as point 3 goes, I don’t see why passive care is an essential component of treatment. As far as the latest research goes, structured exercises and return to sport have significantly more data backing than any passive modality. In fact, I am not even sure if there any passive modalities that had been shown to be efficacious in any decent size randomised controlled trial.

1. Our treatments are 20-30 minutes (at least)

I don’t think treatment time alone is a good indicator of the quality of treatment. But for reference, we do see clients for a minimum of 20-30 minutes. (Way longer than the 5-minute sessions you get at chain chiropractic clinics.) More often than not, we do run over time to 45 mins or even an hour with no extra charge. Our principle is simple – you do not leave our door until we have achieved what we set out to do.

As we mentioned in our previous post, we work on a criteria-based (not time-based) progression. We have a plan for you and it is important to us that we meet the ‘KPIs’ set for each visit. In most circumstances, we discharge our clients from our care between four to seven visits. I promise you no longer chiropractors in Singapore can match that.

2. Treatment is multimodal in nature

Yes, we do offer the largest array of treatment modalities in Singapore – from the standard chiropractic adjustments to IASTM to dry needling, we have it all. However, our practice is very much biased towards exercise and rehabilitation. Research data and personal clinical experience both supports that this is the best approach to manage sport injuries. Since we started eliminating the use of passive therapies, our clients started to see MORE improvements in a shorter period of time. This is how we are able to discharge you from our care in four to seven visits.

3. Treatment should contain active (exercises) and passive components

I think this is probably outdated. For most parts, research and evidence-based clinicians agree that active care supersedes passive components. Chiropractors and physiotherapists who primarily focuses on exercise and movements are on the rise and we are getting results better than our counterparts who still offers manual therapy.

We don’t offer packages at Square One. That in itself should speak volumes.

4. No mandatory x-rays required for treatment

Agreed. Kinda old news. We think this should be the industry/profession standard but since chiropractic is not regulated in Singapore, we have to deal with the substandard chiropractors who are offering services below what is considered best practice guidelines. At the end of the day, we are blogging to help you discern between the chiropractors and the quacks.

5. No predetermined treatment schedules or prepaid contracts of care

We are almost certain we are the only sports chiropractic clinic in Singapore who do not offer packages at all. Let us know if you find some one else who doesn’t offer packages.
